There's a call on the Health Minister to dig deeper into the report of Aoife Johnston's death in UHL to prevent something similar happening in the future.
Limerick TD Willie O'Dea is describing the report as "pretty damning", after it found the 16 year-old's death was almost certainly avoidable.
It shows overcrowding was more severe than it should have been and doctors and nurses were unaware of the risk of sepsis which Aoife was facing.
