Hazard won the silver ball at the 2018 World Cup in which Belgium finished third, but he has struggled for form and fitness in recent years.
Belgium were knocked out at the group stage in Qatar.
Real Madrid player Hazard made 126 appearances for the Red Devils, scoring 33 goals.
