The Bundesliga champions scored a 97th minute equaliser to earn a 2-all draw with Roma to win their semi-final tie 4-2 on aggregate.
Leverkusen will play Atalanta in the decider at Dublin's Aviva Stadium later this month.
The Italian club beat Marseille 3-nil to complete a 4-1 aggregate victory.
