Figures seen by Kfm show that in the CHO 7 region - covering Kildare/West Wicklow and parts of Dublin - nearly 9,400 children did not receive their 9 to 11 month Public Health Nurse assessment between 2020 and 2024.
The worst year locally was 2020, when only 63 percent of eligible children were checked.
Performance improved in 2022, but last year the rate dropped back again, with more than 2,100 missing the check.
Nationally, around 70 thousand children missed the check.
