Convicted paedophile Bill Kenneally has died in prison.

The former basketball coach was serving a 19-year prison term for sexually abusing young boys in Waterford from the late 70s through to the 90s.

The Irish Prison Service says a person in custody died at Midlands Prison this morning.

A report published last week by the Commission of Investigation found a number of failures by State agencies in their investigation of Mr Kenneally.

The Garda response was described as "a clear and serious dereliction of duty, even by the standards of the time".
