His father Vincent - himself receiving cancer treatment - says Evan was “lost in the system".
These images show the results of repeated falls suffered by 15-year-old Evan Gardiner from Celbridge who has been waiting three years for surgery.
The video shows how Evan's ankles have turned in resulting in his falling.
Evan has been a patient in Crumlin since birth and is also connected with Tallaght and Naas hospitals.
His father Vincent Gardiner says he was initially told insoles would be enough to correct his son's walking.
But as Evan grew, his ankles turned in, his kneecaps began to dislocate, and he started falling over regularly.

Evan, who has Down Syndrome, has been left waiting more than three years for vital surgery - despite doctors warning that without it, he could end up in a wheelchair.
His operations have been repeatedly scheduled and cancelled at short notice, leaving his family under immense strain.
His father Vincent - himself receiving cancer treatment - says Evan was “lost in the system".
"We were told Evan was lost in the system. I asked, what do you mean lost? We’ve been at every appointment. Yet sometimes we’d arrive with a letter, and be told we shouldn’t even be there."
He said his son's ankles have turned in, resulting in repeated falls.
"His knees just give way. He could walk fifty steps, or ten steps, and then fall. You can’t tell when it’s going to happen - and he’s falling on a regular basis now.”
He said the strain has taken its toll on his family: "The strain is tough. I had to stop work because I was overwhelmed with it all. We’re a strong family, but this has put huge pressure on us."
"Evan was a really good swimmer, he loves music and dancing. Now a wheelchair has been delivered to the house. He’s a happy, funny young man - but being put on hold like this is just so disappointing.
Kfm understands that an appointment has been scheduled for this week but Vincent said his son has been told this before.
He said he is hopeful that it will go ahead or his son could end up in a wheelchair.
"It’s not just about Evan. There are so many families across the country being let down by our health system," he said.
In a statement to Kfm, a CHI spokesperson said: "Children's Health Ireland cannot provide details about the care provided to individual patients publicly. Even if a family decides to make their story public, we are still bound by patient confidentiality for all patients."
