There are calls for a review into prison security after an inmate died in their cell.
The man died in Midlands Prison in Portlaoise in 2021, and it was revealed that staff were unable to locate the key to his cell while he was having an allergic reaction.
The Office of the Inspector of Prisons said a nurse 'watched helplessly' through the flap of the door, while the 59-year-old man lost consciousness and died.
The key was locked in a separate room, and the staff member responsible for it was on a break.
Speaking on Kildare Today, Niamh McCormack, Legal Policy and Public Affairs Manager with the Irish Penal Reform Trust.
"What's extremely concerning is there was 300 people behind locked doors on that day who wouldn't have been able to be accessed in an emergency situation, because there was nobody there to man the key room."
Ms McCormack also questioned what would've happened if there was a fire, as well as demanding a thorough audit and accountability measures in response to the inmate's death.
